We currently only supply art kits to children battling cancer. Please only submit this form if you are a parent, legal guardian or social worker for the child!
At this time, we can only support children located within the U.S.

Acceptable forms of proof but not limited to:
- Photo of the child/family
- Simple letter from the child's Social Worker or Care Team recommending an art kit
- Screenshot of your social media account where you share the child's cancer journey
